Review of High-Risk Medicaid Programs Finds Weaknesses in Billing, Eligibility and Provider Oversight
SAINT PAUL, MN (September 25, 2026). Minnesota’s legislative watchdog is urging lawmakers and the Department of Human Services to strengthen safeguards across Medicaid-funded services after an outside assessment identified vulnerabilities in 14 programs considered at elevated risk for improper payments.
The assessment, conducted by Optum and summarized by the Office of the Legislative Auditor, examined weaknesses that could create opportunities for fraud, waste or abuse.
The reported vulnerabilities involved eligibility standards, billing and coding rules, provider supervision and information-technology controls. The Legislative Auditor recommended clearer program requirements, stronger fraud-detection systems and technology capable of identifying suspicious billing patterns before claims are paid.
Department of Human Services officials cautioned that a flagged claim is not necessarily fraudulent. The department said some claims initially identified as concerning were cleared after the analytical process was refined to reflect the applicable program rules.
The Office of the Legislative Auditor also emphasized that it had summarized, but had not independently verified, Optum’s underlying findings. The report therefore should not be interpreted as proof that each identified transaction involved wrongdoing.
Those qualifications do not eliminate the report’s central concern. Minnesota has repeatedly pursued providers accused of exploiting publicly funded human-services programs after payments were already issued. Systems that identify unusual billing before payment could reduce both financial losses and the burden of attempting to recover money later.
